All You Mums Out There!?

I am wanting to make a pram quilt with a triangular flap that folds over the pram apron when being used, the sort that just houses like a large cushion pad about 2" deep. How does the little stuffed pad fit into the quilt? Does it slip in the top with a velcro closing or does it split down the middle at the back to pop the quilt in that way.
